Purchasing services from funded non-government service providers
Under the Disability Services Act 2006 and the Commission for Children and Young People and Child Guardian Act 2000, all non-government service providers funded by the Department of Communities, Child Safety and Disability Services (the department) who engage workers must ensure these workers undergo criminal history screening as follows:
- workers who will work only with adults with disability:
- must obtain a yellow card from the department - Application for criminal history screening prescribed notice (yellow card) Form 10-1.
- if they already hold a blue card, they can apply for a yellow card exemption - Application for criminal history screening (exemption notice) Form 10-11.
- workers who will work only with children, including children with disability (aged 0-17 years):
- must obtain a blue card from the Queensland Government Blue Card Services - Blue card application Form BC.
- workers who will work with both adults with disability and children with disability:
- must obtain a blue card and yellow card exemption from Blue Card Services - Blue card application and request for yellow card exemption Form Y. (this form is submitted to the Blue Card Services who will notify the department of the outcome and the department will process the yellow card exemption request; or
- if they already hold a blue card and will work with both adults and children they must obtain a yellow card exemption from the department- Application for criminal history screening (exemption notice) Form 10-11.
If your host provider employs your workers they, as the funded non-government service provider and employer, will complete all relevant applications and documentation.
